La journ\u00e9e de lancement officiel du projet APRECIT intitul\u00e9 \u00ab \u00c9valuation des strat\u00e9gies pour am\u00e9liorer le d\u00e9pistage et la prise en charge globale de l\u2019infection tuberculeuse latente au Cameroun et \u00e0 Madagascar \u00bb a eu lieu le 30 octobre 2020, \u00e0 la Salle de Conf\u00e9rence de l\u2019Institut Pasteur de Madagascar (IPM). En collaboration avec les programmes nationaux de lutte contre la tuberculose (PNLT) de Madagascar et du Cameroun, le projet APRECIT, financ\u00e9e par l\u2019Initiative 5% d\u2019Expertise France, est n\u00e9 de la coop\u00e9ration entre la Fondation M\u00e9rieux,  l\u2019IPM et le Centre Pasteur Cameroun (CPC).<\/p>\n\n\n\n

Cet \u00e9v\u00e9nement avait pour objectif d\u2019informer les diff\u00e9rents acteurs et partenaires du lancement du projet et d\u2019en rappeler les grandes lignes. L\u2019ouverture de cet atelier s\u2019est d\u00e9roul\u00e9 en pr\u00e9sence du  Dr Manuella VOLOLONIAINA  repr\u00e9sentante du Ministre de la Sant\u00e9 Publique,  du Dr Voahangy RASOLOFO, Directrice Scientifique de l\u2019IPM, du Dr Marc BONNEVILLE, Directeur G\u00e9n\u00e9ral adjoint de la Fondation M\u00e9rieux, et de Madame Veronica NOSEDA, repr\u00e9sentante d\u2019Expertise France. Diff\u00e9rentes interventions ont suivies permettant de mieux pr\u00e9ciser les objectifs du projet APRECIT ainsi que les activit\u00e9s qui seront men\u00e9es.<\/p>\n\n\n\n

La tuberculose se pr\u00e9sente sous deux formes, la tuberculose maladie et l\u2019infection tuberculeuse latente (ITL) qui est sans sympt\u00f4mes apparents. La prise en charge de toutes les personnes infect\u00e9es, malades ou asymptomatiques, figure parmi les axes strat\u00e9giques d\u2019intervention des PNLT. L\u2019Organisation Mondiale de la Sant\u00e9 (OMS) pr\u00e9conise la prise en charge pr\u00e9coce des populations vuln\u00e9rables expos\u00e9es \u00e0 une infection par la tuberculose, notamment les enfants \u00e2g\u00e9s de moins de 5 ans et les personnes vivant avec le VIH. Ces derniers ont en effet une probabilit\u00e9 plus \u00e9lev\u00e9e de d\u00e9velopper les sympt\u00f4mes de la tuberculose dans les 6 mois apr\u00e8s leur exposition, surtout lorsqu\u2019ils vivent sous le m\u00eame toit  qu\u2019un patient atteint de tuberculose.  La mise en \u0153uvre des recommandations de l\u2019OMS pr\u00e9sente plusieurs difficult\u00e9s comme l\u2019acc\u00e8s m\u00eame aux populations vuln\u00e9rables, la strat\u00e9gie de d\u00e9pistage ou l\u2019adh\u00e9rence au traitement pr\u00e9ventif d\u2019une dur\u00e9e de 6 mois par un antibiotique.<\/p>\n\n\n\n

Dans le projet APRECIT, le CPC et l\u2019IPM vont assurer la coordination du projet dans leurs pays respectifs pour le recrutement et le suivi de 2500 sujets contacts intradomicilaires, identifi\u00e9s \u00e0 partir de 250 patients ayant une tuberculose. Pour Madagascar, ces patients seront diagnostiqu\u00e9s au niveau du centre hospitalier universitaire (CHU) de Fenoarivo et celui de Soins et de sant\u00e9 publique d\u2019Analakely (CHUSSPA). Des agents communautaires et le personnel m\u00e9dical sensibiliseront les populations et assureront le suivi des personnes au sein des foyers abritant ces sujets dits \u00ab contacts \u00bb. Le personnel m\u00e9dical sera impliqu\u00e9 pour l\u2019accompagnement et le suivi des populations vuln\u00e9rables qui recevront un traitement antibiotique pr\u00e9ventif.<\/p>\n\n\n\n

A Madagascar, 1 500 personnes seront suivies pendant 2 ans dans le cadre du projet APRECIT au niveau de cinq Fokontany de la Commune Urbaine d\u2019Antananarivo et trois Fokontany de la Commune rurale de Fenoarivo.<\/p>\n\n\n\n

Enfin, compte tenu de la pand\u00e9mie actuelle, un volet suppl\u00e9mentaire a \u00e9t\u00e9 int\u00e9gr\u00e9 au projet pour \u00e9valuer l’impact du COVID-19 dans la gravit\u00e9 de l\u2019infection chez les personnes incluses dans l\u2019\u00e9tude.<\/p>\n\n\n\n

Le projet APRECIT permettra d\u2019affiner les strat\u00e9gies de prise en charge des patients atteints d\u2019une tuberculose, en particulier d\u2019une tuberculose latente. Les r\u00e9sultats de cette \u00e9tude op\u00e9rationnelle permettront de proposer aux PNLT des recommandations pour renforcer la prise en charge des populations vuln\u00e9rables.<\/p>","protected":false},"excerpt":{"rendered":"
